Goal Updates and Chair Transitions

Trimester call approaching? Need to update your goals?
Please fill out the GOAL UPDATE FORM for each of your goals (you will need to submit the form one time per goal).
If you have completed a goal and need to submit a new goal, access the NEW GOAL FORM.
Is your Chapter holding an event?
Please submit event information prior to your event, with the EVENT FORM.
If there was a drastic change to the expected numbers of attendees or the event itself, please let us know via the email listed on the event form.
Looking to transition Chapter Chairs?
Please complete the TRANSITION FORM.
You may update any changes to your faculty liaison through the TRANSITION FORM as well.
After you have completed the transition form, schedule a meeting with your Chapter’s past and current Chairs and a COTAD Chapter Committee member by clicking on the button below:
